Yet it cannot be said that it is an absolute or even a relative necessity, since the recoveries recorded have been permanent, even when the simplest methods were used. At the same time, in patients able to bear an operation slightly prolonged. a one of the osteoplastic methods should be used, either by the incision and bending down of the unclosed laminae, or by the use of a periosteum and bone graft from the ilium, (Senenko, ) or ribs, (Botroff, ) supplied with a nutrient flap. The ease and rapidity of application of the Robson and Bayer plastic methods by the use of muscle or fascia, and the permanent security afforded by it to the spinal canal, make it an imperative duty to use one of them whenever justified by the strength of the patient. In favorable cases after the bony defect has been made good, the flaps of fascia should be brought over as an additional protection. Heteroplastic methods are to be absolutely condemned, a priori. The attempt of Mayo Robson to implant a piece of rabbits bone resulted in failure. The demonstration by Barth that pieces of bone implanted in defects of bone serve only as convenient supporting framework for the growth of new bone, and do not themselves live and grow, is proof positive that heteroplastic methods must fail when any important defect of the spinal arches is to be made good. The bone used should be raised with its periosteum, and a nutrient pedicle supplied after the Wagner-Wolf method.
